How Can I Get A Doctor To Change A False Diagnosis?

About 107 days ago, I made a post in legal advice, regarding an incorrect diagnosis I was given.

The advice I received regarding next best steps to get it corrected was useful, namely ... track down the physician who gave the false diagnosis, and go from there.

I have (finally) tracked down the origin of the false information, which happened to be an office visit with a Nurse Practitioner in 2021.

The false diagnosis I was given, was depression, and anxiety. This false diagnosis, which I was wholly unaware of, led to my being (incorrectly) discharged from the military. Consecutive physicians have used this foundational mistake as a springboard to attempt to diagnose me with ever-increasingly-severe mental health conditions, none of which are accurate to my real mental health status.

I have reviewed the medical records, which show that my depression/anxiety screening was at exactly zero. This is also my memory of that time frame, as I was in a wonderful relationship at the time, and very happy. Yet... the very next note from her office, shows "Depression, Anxiety Disorder" as my "past medical history." However, the actual "past medical history" (the previous visit summary note) showed exactly no depression or anxiety.

Moreover, the provider is a nurse practitioner, not a mental healthcare/behavioral health provider, is she even allowed to make a mental/behavioral health diagnosis?

How can I contest this, and get this record corrected?
